Every now and then we have those rare opportunities to participate in a meeting that either substantially changes our perspective on an issue or inspires us to learn more and ultimately to act to make a true difference. I had the opportunity to participate in one of those meetings recently at the AMA Headquarters in Chicago. Led by three leading physician researchers (Drs. Christine Sinsky, Tate Shanafelt, and Mickey Trockel) and sponsored by the AMA along with the Physicians Foundation the day was dedicated to increasing the awareness and implications of physician burnout. The meeting, Joy in Medicine: Multi-stakeholders Conference was designed to bring together 40 plus leaders of various national organizations, researchers, and policy experts to explore a national agenda needed in response to the growing impact of physician burnout.

Here are just a few observations or research conclusions that grabbed my immediate attention:

  • Current estimates suggest that the prevalence of physician burnout now exceeds 50%
  • Burnout is linked to increased self-reported errors, increased physician turnover, higher mortality of hospitalized patients, reduced time allocated to clinical care of patients, and an increased number of unsolicited patient complaints
  • 7% of US workers report working over 60 hours per week but 60% of practicing physicians report routinely working more than 60 hours per week

The literature on the impact of physician burnout is rapidly growing and more is on the way. As medical society executives we have a tremendous opportunity, and I sincerely feel, a responsibility to become aware of the emerging knowledge and to take informed actions to address this issue that is impacting our members.

I remember reading a quote from H.L. Mencken that went something like: “For every problem there is a solution that is neat, plausible and wrong…” Physician burnout is a complex, multiple-faceted problem that will defy simple solutions so as you contemplate how best to respond consider this model from the WellMD Center (Stanford University) that Dr. Mickey Trockel shared during the Joy in Medicine meeting. The WellMD Professional Fulfillment Model promotes the following three dimensions:

Culture of Wellness Organizational work environment, values and behaviors that promote self-care,
personal and professional growth, and compassion for ourselves, our colleagues and our patients.
Efficiency of Practice Workplace systems, processes, and practices that promote safety, quality,
effectiveness, positive patient and colleague interactions, and work-life balance.
Personal Resilience Individual skills, behaviors, and attitudes that contribute to physical, emotional,
and professional well-being.

As medical society executives we will be wise to begin addressing the causes, symptoms, and cures to the issue of physician burnout if we haven’t already. Fortunately, I know that many of you already have begun building and implementing programs designed to protect and restore physician well-being. As you engage in this important work please share your successes as well as your failures with the rest of us. The more we know and understand the more we can truly help our members and ultimately the patients they serve.
